Zeta Tau Alpha, Big Moment on Campus 2023, Contestant Entry Form
★ BMOC 2023 will be held on March 16th at 7:00pm in the TMB Auditorium. 
★ The Deadline to sign up and participate is MARCH 14th. 
★ Your performance may be a solo or group with up to 7 participants, and all must be students at JSU.
 The entry fee is $20 per act, whether a solo or group, and must be received no later than March 9th.
★ We welcome all types of talent - onstage and off. Performance arts, such as music, dance, drama, comedy, athletics, etc., will be live on-stage and may not exceed 3 minutes in length. Off-stage presentations will include art, crafts, woodworking, pottery, fashion design, and so on; and there will be tables available for those contestants to set up a display of their work for judging.
★ There will be prizes for various categories plus a $500 prize for the overall winning presentation.
★You MUST complete all parts of the form below and agree to the Terms and Conditions to participate.
★If we have more live performance entries than can be scheduled for the event, we will need to hold preliminary auditions in advance.

         - Under "I'd like to donate, click on the $20 option. Up and to the right, click on the checkbox in blue to remove the check, so that you will not be charged the fee.
         - Type your name in the "My Name" box. Must match the name that you enter below on this form.
         - In the Comment box - type: "Contestant"
         - Complete the credit card information

All presentations must be approved by the Philanthropy Committee of ZTA before the event and may not be changed or altered after the fact. Failure to follow these Terms and Conditions will result in immediate disqualification.
